Contact Your Insurance Provider:
Ask if your policy has coverage for a cranial prosthesis?
If yes, ask them to confirm coverage for these codes (synthetic Hair A9282 or Human Hair L8499) or if you have your prescription, give them that code.
How much is applied to your deductible?
Are you required to see an in-network provider? If yes, ask for an exception due to having no in-network providers close to home.
Request a claim form and submission details, i.e., address to mail the claim to.
Request that they send you an email with all the details discussed today.
Get a Prescription:
Obtain a written prescription from your doctor for a “cranial prosthesis,” including your diagnosis code.
